Service provision is person-centered and in line with the amount, duration and frequency as referenced in the Individualized Support Plan (ISP). PPHS continuously collaborate with individuals’ team to ensure effective implementation of services as delineated in the ISP and in accordance with “Everyday Lives” recommendations for supporting individuals.
PPHS promotes individual’s participation in community functions and activities and encourage the use of community resources.
PPHS maintains an in-house Behavioral Specialist (BS) who conducts comprehensive assessments and develops behavior support plans for those individuals in the program that require behavioral support services.

The BS trains all direct support staff and monitors the proper use and effectiveness of the plan that includes strategies to support the individual based upon assessment, and the provision of interventions.
The BS will develop a fade plan to systematically titrate restrictive intervention(s) if the use of such is part of an ISP. The BS will revise behavior support strategies as needed, and document all processes.
